extraction utilities for parsing AI responses:
- extractJson<T>(): Multi-strategy JSON extraction
- extractJsonWithKey<T>(): Extract with required key validation

JSON.parse to fail. Added multi-strategy extraction similar to
validate-issue.ts:

reading from phaseModels settings and always used the default haiku model.
